Scalpel blade numbers identify shape, not size. The most widely used are #10 (curved cutting edge, general incisions), #11 (straight pointed blade for stab incisions), #15 (small curved blade for short precise incisions) and #20 (larger curved blade for heavier tissue). Blades numbered in the 10-15 range fit a #3 handle; the 20-25 range fits a #4 handle.
Getting the handle pairing wrong is the single most common ordering error in blade procurement, and it renders an otherwise correct shipment unusable.

The three that account for most volume are #10, #11 and #15. A distributor stocking only those three covers a large share of routine demand, then adds #20-23 for customers doing heavier work.
Handle compatibility: the #3 and #4 rule
Blade and handle fittings are not universal. The simple rule:
- Handle #3 takes blades numbered roughly 10 to 15.
- Handle #4 takes blades numbered roughly 20 to 25.
Handles also come in longer variants for deep-field access, and in angled forms. When ordering, specify both the blade number and the handle number - "#15 blades for #3 handles" leaves nothing to interpretation.
The fitting slot is machined to a tight tolerance. A blade that fits loosely can shift during use, and one that fits too tightly is difficult to mount and remove safely. This tolerance is a genuine quality differentiator between manufacturers, and it is worth testing on samples with the handles your customers actually use.

Safety scalpels and sharps injury
Safety scalpels incorporate a retractable or shielded guard covering the blade when not in active use. Demand for them has grown steadily as healthcare systems have tightened sharps-injury prevention requirements, and in some markets their use is effectively expected rather than optional.
Two broad designs exist: fully disposable single-use units where blade and handle are discarded together, and reusable-handle systems where a guarded blade cartridge mounts to a durable handle.

Blade material and edge quality
Blades are typically made from high-carbon steel or high-carbon stainless steel. High-carbon steel takes an exceptionally keen edge; stainless offers better corrosion resistance. Since blades are single-use, edge sharpness generally outweighs long-term corrosion resistance in the design trade-off - which is why higher-carbon grades dominate. How to assess blade quality on samples:
- Inspect the edge under magnification for consistency and absence of nicks.
- Check the fitting slot against the handles your customers use.
- Confirm packaging integrity across a sample of the batch - a compromised sterile barrier makes the blade unusable.
- Verify batch coding and expiry marking are present and legible.

Blades are a consumable, which changes the purchasing logic. They reorder on a predictable cycle, so the relationship is recurring rather than transactional - which makes reliability of supply worth more than a marginal unit-price saving.
- Concentrate on #10, #11 and #15 for the core range.
- Match handles - stock #3 and #4 handles alongside the blades that fit them.
- Check expiry dating on sterile stock and rotate inventory.
