Surgical instruments are sterilised most commonly by steam autoclaving, but sterilisation only works on a clean instrument - organic residue shields microorganisms and causes staining. The sequence that protects both patients and instrument life is: rinse immediately after use, clean thoroughly, inspect, lubricate hinged joints, then sterilise with ratchets left open.
Most instrument corrosion blamed on poor manufacturing is in fact a reprocessing problem - or the interaction of a marginal instrument with an aggressive but entirely normal cycle.

Why cleaning comes before sterilisation
Blood, saline and tissue residue do two things. They physically shield microorganisms from steam contact, so a soiled instrument may emerge unsterile from a technically correct cycle. And they attack the instrument itself - saline in particular is aggressive toward stainless steel, and residue baked on by autoclave heat becomes far harder to remove.
The single most valuable habit is rinsing instruments promptly after use, before residue dries. Dried blood is materially harder to remove and prolongs the contact time between corrosive residue and the instrument surface.

Ultrasonic cleaning
Ultrasonic cleaners use cavitation to reach serrations, box locks and hinges that brushing cannot. For jointed instruments they are the most effective cleaning method available.
Three rules matter:
- Open all hinged instruments so the joint surfaces are exposed.
- Do not mix dissimilar metals in the same cycle - contact between different metals in solution can cause electrolytic damage.
- Change the solution regularly. A saturated bath redistributes soil rather than removing it.
Autoclave parameters
Steam sterilisation works by exposing every surface to saturated steam at temperature for a defined time. The commonly referenced parameter sets are around 121°C and around 134°C, with the higher temperature permitting shorter exposure. Exact times depend on cycle type, load and your autoclave's validation - follow your validated cycle rather than a general figure.
What matters as much as the numbers:
- Leave ratchets open. A closed ratchet holds the jaw faces in metal-to-metal contact, preventing steam contact and stressing the instrument as it expands. This is the most common and most damaging loading error.
- Do not overload. Steam must circulate; a packed chamber creates cold spots.
- Allow drying. Instruments packed damp corrode and can compromise the sterile barrier.
- Do not stack heavy on delicate. Fine tips deform easily under load.
Why instruments corrode: reading the symptoms

Lubrication
Hinged instruments need lubrication at the box lock to prevent stiffness and wear. Use a lubricant specifically intended for surgical instruments and compatible with steam sterilisation. Do not use industrial oils - they are not steam-permeable, so they can shield surfaces from steam contact and compromise sterilisation.
Extending instrument life
- Rinse immediately; never leave instruments soaking in saline.
- Use distilled or demineralised water for final rinse where possible.
- Open all hinges for cleaning and sterilisation.
- Keep dissimilar metals apart during ultrasonic cleaning.
- Inspect under magnification at regular intervals.
- Retire instruments whose ratchets no longer hold - a slipping clamp is a clinical risk, not a cosmetic issue.
